The movie begins with a narration about Ludlow, a small town where the local forest, known as the Pet Sematary, was once used by a tribe to summon spirits to ward off ancient evil. Despite their efforts, the evil persists, tempting people with promises to resurrect their loved ones.

In 1969, a man is seen burying a corpse in the forest while his dog keeps watch. After the man leaves, the ground begins to move, and the dog is pulled underground by an unseen force.
